PRACTITIONER LISTING INACCURACY REPORTING

In accordance with regulatory requirements, the practitioner listing is validated on a quarterly basis. Reported inaccuracies are promptly investigated, with updates being posted to the practitioner listing within 30 days (or sooner, where required by law).

If you believe there is an inaccuracy in the practitioner listing you have three (3) ways to report the potential inaccuracy. You may send an email to ProviderInfo@ashn.com, call (800) 972-4226 Opt. 4, or complete and submit the electronic form below. Please note this form is for the reporting of potential practitioner listing inaccuracies only; if you have a complaint or grievance, please refer to the Grievance Section at: Resources – Members
